Enrollees must live, work or attend school in either Brevard, Orange, Osceola, or Seminole Counties in order to register for the DUI program with Florida Safety Council, Inc. Please be aware also that you have 90 days from the on-line payment date to complete your registration, assigned class and evaluation. Failure to do so will result in loss of your fee payment and notification to DHSMV of your failure to complete the DUI program.

PROPER COURSE ASSIGNMENT IS CRITICAL: The assignment of Level I or Level II is ultimately the responsibility of the DUI Program. It is very important that you read the information carefully and respond as honestly as possible regarding your Level Assignment. DUI School Level assignment is based on the number of lifetime arrests and/or DUI School qualifying events. Rulings in courts regarding prior offenses usually do not affect your proper Level assignment. If you have any questions, please contact our customer service prior to enrolling. United Safety Council, Inc reserves the right to determine the proper DUI School Level at any time. You will be held responsible for full payment based on the proper Level assignment and the loss of any fees previously paid, and or services rendered based on false or incomplete information. Your completion certificate may be held or forfeited once false or incomplete information is discovered.

You will be required to complete your enrollment in person and provide the traffic citations (ticket) for the qualifying offense, the arrest affidavit (report), crash report if applicable, an official lifetime driving history (may be obtained through USC Registration Office), and in-person completion of an approved written assessment. Once USC obtains all these documents you will be allowed to complete a scheduled face-to-face evaluation and DUI class.

The Level I DUI course is for first time offenders who have been charged with a DUI, Refusal, or any other qualifying offense OR have never attended a Level I DUI Course before for any reason at any time in your life.

The Level II DUI course is for persons who have previously attended a Level I DUI course at any location at any time in your entire lifetime or have been charged and/or convicted two or more times of an offense requiring DUI school attendance. This includes first time offenders who have been court ordered to complete a Level 2 DUI program.

Change From Level I to Level II DUI Program

All offenders registering for the State of Florida's licensed DUI Program are required to adhere to requirements set for in Florida's Administrative Code (FAC) 15A-10 and Florida Statutes. The requirements imposed by the courts may be different than the requirements set forth in FAC 15A-10. Court requirements may be added to the requirements of FAC 15A-10 but they may not be a substitute. (I.e. The court states you should complete Level I but FAC 15A-10 requires you to complete Level II. In that case, Level I cannot be substitute for Level II.) Therefore, it is vitally important you enroll in the proper Level. Failure to do so, will only delay completion and possible cost you more money and time.

By continuing with this enrollment, I acknowledge that I have read and understand the importance of providing accurate information during DUI Program registration. I agree to cooperate with USC to ensure appropriate DUI Program attendance/completion information is reviewed. I understand that my failure to provide all necessary information for registration will result in payment of the full fee to enroll in the Level II DUI Program and absolutely No Refund of the initial Level I registration will be provided.

Failure to complete all or part of the program (class and evaluation attendance) within 90 days of payment (registration) will result in notification to the Department of Highway Safety and Motor Vehicles of failure to complete and forfeiture of all fees paid. You will be required to re-enroll at full fee and complete all components again regardless of previous completion.

Please be advised that information regarding drug and alcohol patient records is protected by Federal Confidentiality Rules (42 CFR Part 2). The Federal rules prohibit Florida Safety Council, Inc from making any further disclosure of this information unless further disclosure is expressly permitted by the written consent of the person to whom it pertains or as otherwise permitted by 42 CFR Part 2. A general authorization for the release of medical or other information is NOT sufficient for this purpose.

 

Level I registration fee is $338.45* and Level II registration fee is $504.05*. These fees include a Florida state assessment fee ($15.00), a Driver Risk Inventory fee ($5.00), an online processing fee ($7.00) and a service fee ($11.45 or $17.05 respectively). *Florida Safety Council reserves the right to impose additional charges for applicable out-of-state driving records. The full registration fee above is due on the final page of this registration. Payment of fees is acknowledgement and agreement to the program rules and regulations. All fees payable to Florida Safety Council, Inc are non-refundable, even if you are found not guilty of the DUI and your administrative suspension is invalidated.
